Life Coach – (LPC, LCSW)
Provide proactive outreach, engagement, health education including bio psychosocial support, referral and coaching tailored to military personnel. This coaching is provided through telephonic or Skype sessions. Assessment of needs and barriers to goal accomplishment are addressed with a collaborative plan for success.
Key Responsibilities
- Employs biopsychosocial motivational interviewing techniques in the engagement and coaching phases.
- Provides goal setting, condition education, self-coaching and education on coping skills at various phases of interventions and delivered within the framework of approved protocols and assessments
- Develops individualized, educational plans in a timely manner.
- Follows-up with beneficiaries regarding their health plan to support beneficiary's continued needs.
- Documents accurate and complete information for all program activities through the medical management on-line system and other software systems.
- Refers individual to appropriate military clinical facility if issues presented are of clinical nature requiring intervention.
- Collaborates with military clinical facility to ensure seamless transfer of care.
- Reports any adverse incident as directed by protocol.
Education & Experience
Required
- Minimum 2 years of experience
- Current unrestricted license in appropriate state to include:
- Licensed Clinical Social Worker
- Licensed Professional Counselor
- Licensed Mental Health Counselor
- Licensed Marriage and Family Therapist
- Licensed Registered Nurses with Mental Health Background
- Licensed Clinical Psychologist
- Life Coach with credential from International Coach Federation plus 2 years’ experience in providing integrated, biopsychosocial, life coaching services and telephonic mental health outreach services.
Preferred
