Major Duties:

The position is assigned to the Behavioral Health Care Line. The incumbent is a member of the Bio-psychosocial Rehabilitation Services Program that emphasizes the Recovery Model of Rehabilitation, incorporating Evidenced Based Practices in programming and recovery plan development, to include motivational interviewing, CBT, CPT, Seeking Safety modalities, and other EBP components.  
 
The incumbent of this position provides behavioral health programming and services to Veterans in Recovery, coordinating their care to include substance abuse services, mental health services, vocational services, assistance with homeless outreach services, and collaboration with other disciplines/treatment modalities to ensure that the Veterans’ recovery plans are self-directed and patient centered.  
 
The incumbent functions as a member of an interdisciplinary teame setting. The patients for whom the incumbent regularly provides treatment services have a wide range of health care problems including acute and chronic, severe and debilitating conditions, physical and psychiatric conditions, substance abuse or homelessness. Incumbent employs a full range of behavioral health treatment modalities with individuals, families and groups with appropriate regard for the complexity of a given problem as assigned; he/she provides services at any and all points on the continuum of care.

The incumbent must demonstrate the knowledge of the changes associated with aging and possess the ability to provide care based upon age-related factors as noted in age specific competencies. The incumbent focuses on the patient in the context of family and community by integrating an assessment of living conditions, individual and family dynamics, resource needs and cultural background into the patient's plan of care.
 
Tour of Duty: The hours of operation may vary Monday through Friday, 8:00 a.m. - 4:30 p.m. EST. Hours may change at the discretion of the program.
 
Duty Location: VAMC Canandaigua, New York. The location may change at the discretion of the program.
