Bereavement/Spiritual Care Coordinator
Northland Hospice & Palliative Care – Flagstaff, AZ
The Bereavement/Spiritual Care Coordinator is responsible for determining a level of support and care to families before and following the death of a hospice patient. The goal of the Coordinator is to help the patient, family, and significant others, to provide tools to help cope with grief related to death and loss. Develop bereavement/spiritual care plan via counseling sessions, group sessions, and organize cards/letters, memorial services, and other assistance programs. Bereavement services to family/significant others will be provided for at least 12 months and may regularly go longer, as deemed necessary by risks and other situational factors. The Bereavement/Spiritual Care Coordinator will work closely with the interdisciplinary team to coordinate needs and services of family members.
Minimum Requirements:
-Degree in Social Work or Bereavement Counselor certification/counseling degree. Master’s degree preferred.
-Experience with hospice, families/caregivers and patients dealing with grief or loss.
-Ability to establish positive relationships with clients from a variety of cultural, religious, and socio-economic backgrounds.
-Knowledge of community resources that may be used for appropriate referrals of bereaved family members.
