Job Description
Purpose: The Pastoral Counselor is that member of the Hospice Interdisciplinary Team who directly provides spiritual support to patients, families and staff.
Responsibilities:
1.Provides pastoral counseling services to patients and families to meet the needs of their spiritual beliefs and desires.
2.Assists with bereavement counseling through letters, phone calls, visits and gatherings of Hospice Family Care friends and families.
3.Facilitates visits by local clergy, pastoral counselors and other individuals who can support the patients’ spiritual needs to the best of their ability.
4.Attends Interdisciplinary Group Conference to review, update and assist in assessing the spiritual needs of the patient and family.
5.Actively participates in the coordination of all aspects of the patient’s hospice care, in accordance with current professional standards and practice, including participation in ongoing interdisciplinary comprehensive assessments, developing and evaluating the plan of care, and contributing to patient and family counseling and education.
Required Skills:
1.Express an excellent understanding of grief and loss as well as the phases of dying and bereavement
2.Be appropriately ordained, licensed and/or certified as a chaplain.
3.Must have own reliable transportation, a valid driver’s license and proof of adequate automobile insurance.
4.Understanding of computer usage is required; understanding of Microsoft applications is a plus.
Required Experience:
1.One (1) year experience working with patients and/or families in a crisis situation.
